Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“There was a cabinet in the infant room with a child lock that was broken, and the cabinet had cleaning supplies in it that were accessible to children. This was corrected during the inspection when the supplies were moved to a new cabinet until the lock could be fixed.”
“While watching video for an investigation, children were observed not washing hands after diaper changes and then sitting down to eat without washing their hands.”
“While watching video for an investigation, caregivers were observed completing diaper changes without washing their hands between changes.”
“The director had not reviewed the recall list.”
“An infant class with a six-month-old was combined with a toddler class where the oldest child was three years old. This was corrected during the inspection when the infant class went back inside until the toddler class was done on the playground.”
“Out of six children's files reviewed, one was missing healthcare information and three were missing health statements.”
“The 3- and 4-year-old classes did not have 90 minutes of physical activity scheduled.”
“Out of six staff files reviewed, one staff was missing 20 hours of training for the 2024-2025 training year, and two staff members did not have any training for the 2024-2025 training year.”
“There was a mop bucket with water in it stored in a bathroom that was accessible to children. This was corrected during the inspection when mop bucket was emptied.”
“The chain link patches over gaps in the fence were starting to come loose, resulting in loose wires protuding from the fence.”
“The director stated that the school-age children were using the field next to the operation, which had not previously been used or measured. This was corrected during the inspection when the director stated that they would no longer use the field.”
“During the investigation, there was sufficient evidence found to determine that caregivers were not adequately aware of children's ongoing activities as a child in care was able to capture a mouse on the playground and was bitten.”
“There was a hole in the fence of the playground that was large enough for a child to crawl through.”
“Out of six children's records reviewed, three were missing physician and emergency medical facility information, two were missing a parent's signature for consent to emergency medical treatment, one was missing hearing and vision screening results, and one was missing a parent's acknowledgment that they had received the operational policies.”
